Register With The Practice (New Patients)

New patients wishing to join this practice should print off the New Patient Registration Form  and bring it into the Practice with a form of photo identification. New patients are asked to print off and complete a simple lifestyle questionnaire New Patient Questionnaire, which is used to give the practice a brief outline of your past medical history, and where appropriate, an appointment will be organised for a New Patient Medical, with our practice nurse within one month of registering.  This health check gives the practice the opportunity to meet you and to tell you about the practice, and allows the Practice gather any relevant aspects of your medical history before your records become available to us.

Book a Home Visit

Home visits are arranged for patients to be seen at home who are too ill or infirm to be seen at the consulting room. Home visits are expensive in doctors’ time, so consultation at the Health Centre is greatly appreciated. The home visit rounds are planned before 10.30am and therefore an early request is appreciated. Home visits can be requested by telephoning 01465713343 or by getting a representative to report to our reception desk in the Health Centre. It is most helpful if our staff can be given some idea of your symptoms. This is required in order that the urgency of each call can be assessed by the doctor. We do appreciate your help in this matter.

Between 8.30am and 5.30pm there is always a doctor on call for emergencies. The on call doctor carries a mobile telephone so they can be immediately contacted.

NHS 24 is a telephone health advice and information service, designed to meet your needs, whatever time of day or night.

Evening Service

The doctors in Girvan are members of the Ayrshire Urgent Care Service (AUCS), formerly known as Ayrshire Doctors on Call (ADOC), which is linked to the NHS 24 service.

If you require urgent medical treatment or advice outside the normal working hours of Monday to Friday 5.30pm to 8.30am and all day at weekends, you will contact the out of hours service (NHS 24) on: 111 and a nurse advisor will assess your symptoms, and will advise you what to do next.

  • Via the Managed Repeat Scheme run by the local Pharmacies
  • Via the NHS Chronic Medication Service *

Patients having long-term drug treatment can usually be given repeat prescriptions, without seeing the doctor, but your doctor will wish to review your medication from time to time and you will be then asked to make an appointment. Prescription requests will be dealt with as timeously as possible, but

  • A minimum of 72 HOURS notice is required for repeat prescriptions to be sent to the chemist.
  • 48 HOURS notice is required if prescription is to be collected at the Health Centre.
  • If prescriptions are required in an emergency, please attend the Health Centre in person before 11.00am and we will do our best to have your prescription ready within 24 hours of request.

Computerised Practice
The practice is computerised to ease administration and to provide prescriptions as rapidly as possible. All records, whether on file or in the computer are confidential. In order to maintain confidentiality, test results will only be given to patients themselves or to parents of minors, if appropriate, and at the doctors’ discretion.

To change your name or address, please print and complete the Change of Name and/or Address Form and return to the surgery.
